The Path to Nurse Registry Licensure in Florida
Getting a Florida nurse registry licensed follows a clear sequence. Here is what each stage involves, what AHCA expects, and where to find the official forms.
This is your registry — and your survey.
AHCA will interview you and your team, not your consultant. Surveyors expect your Administrator, Alternate Administrator, and staff to know your policies, your forms, and the requirements behind them, and to answer questions without outside help. Learning and understanding this material is your responsibility. That's why guided preparation is key. A good coach helps you get there, but can't do it for you.
Get Set Up
Before you apply
- Form your business entity
- Secure your office. AHCA requires proof of local zoning compliance with your application.
- Identify your Administrator, Alternate Administrator, and RN
- Read the statute, the rules, and the G-Tags
Build & File Your Application
Your application package
- Complete the application and addendum
- Complete your Proof of Financial Ability to Operate. It is filed with your application.
- Complete background screening
- Prepare your Comprehensive Emergency Management Plan (CEMP) and submit it to your county health department, which has up to 90 days to review it
Get Survey-Ready
Before your initial survey
- Put policies, procedures, and forms in place for every G-Tag
- Assemble sample client and caregiver files
- Keep your CEMP on file and ready to show
- Make sure your Administrator and Alternate can explain your registry's practices
Your Initial Survey
Licensure decision
- AHCA must see your registry in compliance during the survey before a license is issued
- You may not begin operating until your license is issued
- AHCA approves or denies within 60 days of a complete application, which includes satisfactory survey results
How the Package Follows This Path

We work through any AHCA omission letter and review your policies and documentation.
You leave on track for approval and survey.
Session 3: Mock Survey
I ask the questions a surveyor would. You and your team answer.
You leave knowing how to answer on your own.

We coordinate your survey date in advance so I'm available by phone for questions.
